I enclose £.............. payable to 'Joint Clubs Committee'. I agree to be bound by the IYRU Racing Rules 1993-96, the RYA Prescriptions and the Sailing Instructions.
I certify that there is no modification to the yacht or dinghy's hull or rig since it was last rated or that would make it unrepresentative of its class. (If you cannot properly sign this please add a letter of explanation).
I acknowledge that it is my sole and inescapable responsibility to ensure the safety of the yacht and all persons onboard, that the yacht is fully sound and seaworthy and carries the required safety equipment in good and working order, and I declare that the required third party insurance will be maintained in full force and effect throughout the week. The Sailing Instructions and content of the Burnham Week Programme in no way limit this responsibility. I am aware that IYRU fundamental Rule B states that it shall be the sole responsibility of each yacht to decide whether or not to start or to continue the race.
